Class B Ex C: Field Project: Wound Care in Diabetic Foot Ulcer (DFU) Patients

FIKES UPNVJ - This field project is the final assignment for the wound care elective course which aims to enable students to carry out direct wound care on patients by practicing all the understanding they have learned in taking the Wound Care elective course. This project is carried out for 4 treatments on a patient without charging a penny. The following is a series of wound care project activities that we have carried out.

FIRST VISIT (Tuesday, September 26 2023 15.30 – 17.00)
On Tuesday, September 26 2023, we, 5th semester students of Bachelor of Nursing, carried out our first visit to patients on Jl. Warakas V Gang 14 No 80 C RT 10/ RW 13, Warakas Village, Tanjung Priok District at 15.30 – 17.00 WIB. At the first visit we carry out BHSP which is followed by informed consent and an assessment of the patient's wounds by exploring subjective and objective data to determine the appropriate intervention for the patient. Because the patient had a history of diabetes mellitus, before wound treatment began we measured the patient's blood sugar level first and obtained a result of 591 mg/dl. From these results it was stated that the patient's blood sugar was very high.

Then we perform wound care on the patient. Treatment uses 3M wound care principles, namely washing the wound, removing necrotic tissue and choosing the right dressing. The patient's wound is washed until clean, then the necrotic tissue is removed and the wound is closed using an advanced dressing. Because the wound looked dry and had cavities, we chose to use Regular Metcovazin to prepare the wound tissue and use hydrogel for the dry parts of the wound.

SECOND VISIT (Friday, September 29, 2023 11.00 - 13.30)
On Friday, September 29 2023, we, 5th semester students of Bachelor of Nursing, continued our second visit to patients on Jl. Warakas V Gang 14 No 80 C. RT 10/ RW 13, Warakas Village. Tanjung Priok District at 11.00 - 13.30 WIB. Before the second visit wound care begins, we measure the patient's blood sugar level first because the patient has diabetes mellitus. The GDS results at the second visit showed lower numbers compared to the GDS results at the first visit. However, the GDS results are still relatively high, so we re-educate patients to maintain their diet and regularly take diabetes medication according to doctor's recommendations.

Then we perform wound care on the patient. At the second visit, granulation tissue began to appear in the patient's wound. Treatment uses 3M wound care principles, namely washing the wound, removing necrotic tissue, and choosing the right dressing. The patient's wound is washed until clean and then the necrotic tissue is removed. This is then followed by a second visit assessment of the patient's wounds by exploring subjective and objective data to determine the appropriate intervention for the patient. The wound is closed using an advanced dressing. Because the wound looked dry and had cavities, we chose to use Regular Metcovazin to prepare the wound tissue and use hydrogel for the dry parts of the wound.

THIRD VISIT (Tuesday, October 3, 2023 11.00 - 13.30)
On Tuesday, October 3 2023, we, 5th semester students of Bachelor of Nursing, continued our third visit to patients on Jl. Warakas V Gang 14 No 80 C RT 10/RW 13, Warakas Village, Tanjung Priok District at 11.00 - 13.30 WIB. On the third visit we checked our blood sugar first before carrying out wound treatment and the results were 398 mg/dl. These results decreased if seen from the GDS results at the first and second visits.

Then we perform wound care on the patient. During the wound assessment at the third visit, granulation tissue and epithelialization were visible. There is dried blood at the edge of the wound. Then proceed with treatment using 3M wound care principles, namely washing the wound, removing necrotic tissue, and choosing the right dressing. The patient's wound is washed until clean, then the necrotic tissue is removed and the wound is closed using an advanced dressing. Because the wound looked dry and had cavities, we chose to use Regular Metcovazin to prepare the wound tissue and use hydrogel for the dry wound.

FOURTH VISIT (Thursday, October 5, 2023 11.00 - 13.30)
On Thursday, October 5 2023, we, 5th semester students of Bachelor of Nursing, continued our fourth visit, which was our last day treating wounds on patients on Jl. Warakas V Gang 14 No 80 C RT 10/RW 13, Warakas Village, Tanjung Priok District at 11.00 - 13.30 WIB. On the fourth visit, we checked our blood sugar first before carrying out wound treatment and the result was 462
